Stockinette. Based on preference or orders from the physician, you can either have the patient standing or laying down with the leg propped up at the ankle with a stand. Slide the stockinette over the ankle and roll it up to just above mid-thigh. Make sure there are no wrinkles in the stockinette. To prevent wrinkling, cut out the stockinette in the popliteal area. Start wrapping the cast padding about 1 to 2 inches above the medial and lateral malleoli, wrapping 2 to 3 layers at this starting point. Continue up the lower leg using a 50-50 pattern. Wrap around the knee and up the thigh in the same fashion. Ensure the patient's leg remains straight. Wrap three times at the top of the leg. Place an extra strip of folded padding over the patella. Based on patient leg size, you may want to add a second strip of folded padding over the patella. Wrap one more full layer of cast padding over the leg in a 50-50 pattern. Place the bumpers at each end of the padding and fold the stockinette over the bumpers. Note that bumpers may also be added when wrapping the padding by making three turns at each end while applying the padding. Fold over the stockinette. Now we're ready to apply our fiberglass. Wrap cast tape or fiberglass starting at the distal end. Wrap up the lower leg around the knee. Make a fan fold to strengthen knee support if necessary. And up the thigh to one to two inches before the cuff or bumper. Make sure the patient is maintaining a straight leg position. Apply one more layer of fiberglass over the leg. Mold the fiberglass as it hardens, using your palms, not your fingertips. You can make a mold at the back of the thigh to ensure the cast stays in position. An optional step is to mold the cast at the condyles to keep the cast from sliding down the leg. Make sure that mold with your palm of your hands, not the fingertips. And there you have your cylinder cast. To remove the cast, use a marker to draw along the lateral and medial sides of the cast, drawing down the leg to the ankle and then out to the toes.
