Ensure the patient's foot stays at a 90-degree angle by having him or her sit on a higher surface than you and placing the injured lower extremity on a footstand. Put the stockinette on so it is along the tibial tuberosity and coming down 2 inches past the toes. Cut out the stockinette on the front of the ankle. Apply the cotton padding. Begin wrapping the padding mid-foot and come down to the end of the toes. Then wrap back up the foot and ankle and up the leg. Wrap three times around the top of the leg for the bumper. Place a folded strip two layers thick of padding over the tibia. Wrap padding over the folded strip, wrapping from the top of the leg down. Place a folded strip two layers thick of padding under the heel of the foot and covering the medial and lateral sides of the ankle. Make sure the heel is well padded. Then do a triple layered strip of padding for the back of the foot before continuing wrapping the padding down the leg and over the foot. and tuck that in. Take our fiberglass Apply the fiberglass Begin wrapping mid-foot and wrap to the end of the toes Then wrap back up the foot and ankle and up the leg We're going to trim the toes out. Cut out the padding and fiberglass covering the toes. Check to be sure we got good clearance on our baby toe. On the bottom, separate the fiberglass and padding and only cut the fiberglass on the bottom. Mold the arch of the foot. Continue wrapping fiberglass up the leg, leaving about one inch between the fiberglass and top of the cast padding. Then wrap the fiberglass back down the leg. We're going to cut the slit at the top on both sides, fold that over. Cut a slit on each side of the leg and fold the stockinette and padding down over the fiberglass. Now we're going to wrap the splint on. Place the splint behind the ankle and under the foot, and wrap it on with a roll of 3-inch fiberglass. Cut two slits on each side of the splint under the heel, so that you can tuck it in and wrap the fiberglass over. Trim the splint so it lines up with the bottom of the cast. Then fold the stockinette back. Make sure the cast covers the metatarsal heads on the bottom, but the toes have full clearance on top. You want it to come out underneath the metatarsal heads on the bottom, but you want to have full clearance on top. Then you take your cast off. Then you take your collar roll and finish it off. And that's a short leg walking cast without a toe plate. To remove the cast, cut along the lateral and medial sides of the cast, all the way down the leg to the ankle and to the toes. Follow the lines drawn in the video.
