♪ Music Playing ♪ Have the patient bend the knee as instructed by the physician, 30 degree or 45 degree angle, and bend the ankle close to 90 degrees. Place the first piece of stockinette from the top of the patella to just above mid-thigh. and place the second piece of stockinette from just below the patella to pass the toes. Make a fold in the top crease of the ankle. Cut away the fold in the top crease of the ankle. Begin wrapping cast padding three times around the toes for a bumper. Continue wrapping using 50-50 coverage to mid-thigh. I'm going to keep rolling up the leg, and roll as high on the thigh as you can get. Make three turns at the top of the thigh for a bumper. Apply a second 50-50 cotton padding layer. Place extra padding around the heel and ankle. You can use a double-layer strip of padding for a stirrup under the heel. And a double-layer strip of padding for the back of the heel. Then we're going to take a 4 by 38 splint. I'm going to round the top edges. Wet the splint and wring out as much water as you can, then apply to the posterior side of the leg, about one inch from the edge of the padding at the top of the thigh. Wrap with cast padding or gauze to hold the splint in place, stopping at the ankle. Cut a slit on each side of the splint at the heel. And then I'm going to cut the length of the splint. And cut the length of the splint per the physician's order. And you want it to come under the foot to support the metatarsal heads. Wrap the foot and ankle with cast padding or gauze wrap, folding in your cut at the heel. Cut two slits opposite of each other at the top of the thigh and fold back the padding. Cut two slits opposite of each other at the toes and fold back the padding. Then fold the stockinette back over the padding. Make sure the leg is kept at the desired angle until the splint hardens. Finish by wrapping with elastic bandages or colored adhesive self-adhering wrap.
