It's going to cover that side. You just want to come up around the ring and small finger. Take a one-inch piece of stockinette and cut up three-quarters of both sides. Place the stockinette over the second and third fingers. Begin wrapping the cast padding at the wrist and work your way up through the web space and around the second and third fingers. Make three turns around the second and third fingers. Do not allow the fingers to overlap. Come back across the back of the metacarpal. Make sure you got good padding back there. Continue wrapping proximally using 50-50 coverage. Make three turns at the top of the cast for a bumper and continue to wrap distally towards the wrist. We're going to take our splint and do some measuring. Fold splint in half. Measure and mark the length at the base of the thumb. The length of the splint should be... Cut up the middle of the splint to the mark. The material should look like a Y shape. So it looks like that. Then round the edges. Wet the splint and wring out excess water. So take your splint, set it over your fingers. I need to cut it up a little bit higher. Place the splint in the web space of the first and second fingers and around the second and third fingers. Begin wrapping the second and third fingers using gauze or cotton padding to hold the splint in place. On the dorsal side of the hand, bring the splint straight down. On the palmar side, bring the splint in slightly towards the thumb. You may want to cut a small indent for the thumb. Continue wrapping proximally, keeping the splint in place on both sides. Like so. On this one, if you have a fracture that's unstable, you can also put part of your splint here to firm that up so that you just get better control on that radial side. Mold the splint by pressing the second and third fingers down so they are at a 90-degree angle or as far as the patient can go. Keep the wrist in a neutral position or at a 20-degree to 30-degree angle. Cut your slit here. Cut two slits opposite of each other at the fourth and fifth fingers. Fold the stockinette back. Fold the stockinette back over the second and third fingers. Cut two slits opposite of each other at the proximal end of the padding. Fold the padding and stockinette back. Using the elastic bandage, start at the wrist and wrap towards the top of the splint to hold the splint in place. And that's a radio gutter splint.
