Pdf Summary
This document outlines critical guidelines for accurate coding and documentation in pelvis and hip surgeries, emphasizing compliance and revenue protection. It advises strengthening operative notes by clearly stating medical necessity, including failed conservative treatments, imaging results, and activity limitations, while noting prior surgeries to support appropriate coding for conversions or revisions. For Femoroacetabular Impingement (FAI) cases, it stresses verifying documented conservative treatment for at least three months, recording alpha angles, positive impingement signs, and confirmed labral tears, and ensuring no advanced arthritis or payer age restrictions exist to justify surgery.

To prevent arthroscopy bundling errors, only one code (either 29915 or 29916) should be reported when procedures target the same acetabular region, and billing for debridement or synovectomy alongside FAI procedures should be avoided. Where no specific CPT code exists for arthroscopic procedures, code 29999 should be used. In total hip arthroplasty coding, primary cases use code 27130, conversion due to prior incisional surgery use 27132, and revisions are reserved only when prosthetic components fail.

In infection staging, differentiating between articulating and static spacers is vital, with articulating spacers requiring revision codes and static ones coded as 27091; modifier 58 should be appended for staged returns during the global period. For maximizing reimbursement, assigning coders by payer rather than surgeon, reviewing Explanation of Benefits (EOB) for denial patterns, and customizing appeals are recommended. Common high-risk coding errors include improper use of modifier 52 to convert open to arthroscopic codes and billing tenotomy codes when tendons are not fully severed.

These guidelines were presented in the AAOE webinar "Mastering the Changes: From Code Updates to Compliance — Session 3: Pelvis/Hip," delivered by Margie Vaught on March 11, 2026.
